Guillain-Barré Syndrome Recovery: The Role of Physiotherapy
Guillain-Barré syndrome (GBS) is a rare but serious autoimmune condition in which the immune system attacks the peripheral nerves. It typically begins with weakness and tingling in the legs, which can progress rapidly to affect the arms, trunk, and breathing muscles. Most patients reach their worst point within two to four weeks, after which a slow recovery begins.
The recovery from GBS is driven by nerve regeneration — nerves repair and remyelinate at approximately one millimetre per day. This means recovery is slow, often taking months to years, and requires patient, consistent rehabilitation.
In the acute phase, physiotherapy focuses on preventing complications. For patients who are ventilated or severely weak, this includes: regular repositioning to prevent pressure sores, passive range-of-motion exercises to prevent contractures, respiratory physiotherapy to maintain lung function and manage secretions, and splinting to maintain joint position.
As recovery begins, rehabilitation progresses through stages. Gentle active-assisted exercises are introduced, gradually increasing to active and then resisted exercises as strength returns. Functional training — rolling, sitting, standing, walking — is reintroduced progressively. Balance and coordination exercises address the sensory and proprioceptive deficits that often persist.
Fatigue is a common and often persistent symptom. Energy management strategies help patients pace their recovery without overexertion.
Most GBS patients achieve a good or full recovery, though some residual weakness, fatigue, or sensory changes may persist.
