Physiotherapy for Parkinson's Disease: Managing Movement Symptoms

Parkinson's disease is a progressive neurological condition caused by the loss of dopamine-producing cells in the brain. The four primary motor symptoms — tremor, rigidity (stiffness), bradykinesia (slowness of movement), and postural instability — significantly affect mobility and quality of life. While medication is the primary treatment, physiotherapy plays an essential role in managing these symptoms.

Growing evidence shows that regular, intensive exercise has genuine neuroprotective effects in Parkinson's — potentially slowing disease progression. The Parkinson's Foundation recommends at least 150 minutes of moderate-intensity exercise per week. This makes physiotherapy a central component of Parkinson's management from the point of diagnosis.

Physiotherapy for Parkinson's addresses several key areas. Gait training tackles the characteristic shuffling walk, reduced step length, and freezing episodes. Cueing strategies — visual (lines on the floor, laser pointers), auditory (metronomes, rhythmic counting), and mental (imagining stepping over an obstacle) — can effectively overcome freezing.

Balance training addresses the postural instability that increases fall risk, particularly in later stages. Strength training maintains the muscle power needed for posture, transfers, and walking. Flexibility exercises combat rigidity. Dual-task training addresses the common problem of deterioration when walking and thinking simultaneously.

Treatment must be timed to coincide with "on" periods (when medication is most effective), and adapted as the condition progresses.
