Seamless rehabilitation at home following hospital discharge, getting you back on your feet safely.
Hospital deconditioning refers to the rapid loss of muscle strength and fitness that occurs during hospital stays — older adults can lose up to 5% of muscle strength per day of bed rest. Post-discharge physiotherapy reverses this deconditioning and reduces readmission risk.
Who This Service Is Suitable For
Patients recently discharged from hospital after surgery, illness, or medical events who have experienced loss of strength, mobility, or confidence during their stay.

- Regaining safe mobility
- Managing stairs and transfers
- Returning to everyday routines
Arranging Physiotherapy for a Relative?
Looking for physiotherapy for a parent, partner or relative? You are welcome to arrange the initial call on their behalf. With the patient’s agreement, family members and carers can be involved in home sessions and shown how to provide appropriate support between visits.
The patient’s consent and preferences always remain central to every decision.
Your First Visit, Step by Step
Knowing what to expect helps you feel prepared. Here is what typically happens at your first home appointment.
- How long it lasts
- Around 60 minutes, in your own home.
- What we ask about
- Your medical history, current symptoms, medications, any recent scans or hospital stays, and what you would like to achieve.
- Assessments we may complete
- Strength, mobility and stamina, walking and transfers, and how you are managing day-to-day since discharge.
- Does treatment start straight away?
- Yes — most people begin hands-on treatment and their first exercises during this visit, where appropriate.
- What you receive afterwards
- A written exercise programme, clear advice on what to do between sessions, and a summary of your agreed goals and plan.
- Can a family member or carer attend?
- Yes. A family member or carer is welcome to attend — just let us know when booking.
- Clothing & preparation
- Wear comfortable clothing. Have your discharge summary and a list of medications ready.
Practical Questions, Answered
A few things people often want to know before getting in touch.
Can a relative arrange the appointment?
Yes. Family members often arrange the initial call on behalf of a parent, partner or relative. We will speak with the person receiving treatment (or their representative) to confirm what they would like.
Can a carer or family member be present?
Always. A carer or relative is welcome to attend every session, observe, and learn how to support exercises safely between visits.
What if I have limited mobility?
That is exactly who we visit. Treatment takes place in your own home, so there is no travel, no clinic transfer, and exercises are built around the furniture, stairs and spaces you use every day.
Do I need to provide any equipment?
No. Your physiotherapist brings all assessment and treatment equipment to every visit. We simply need a clear, well-lit space to work in.
Can you communicate with my consultant, GP or other clinicians?
Yes, with your consent. We can liaise with your GP, consultant or hospital team and share relevant updates so your rehabilitation fits within your wider care.
How quickly can treatment start?
Same-day or next-day appointments are often available, subject to capacity. After your free phone consultation we will agree a convenient time for your first home visit.
What does the first appointment cost?
Your first visit is £110 and lasts 60 minutes, including the assessment and first treatment. Follow-ups are £99 for 60 minutes, and a package of five visits is £450. Online video consultations are £45 for 30 minutes. Travel is included, and you pay after your appointment by card, bank transfer or cash.
Home Post Hospital Rehabilitation Physiotherapy: Treatment & Rehabilitation
Recovery After Hospital Discharge
Leaving hospital after an illness, surgery, or medical event can be a vulnerable time. Many patients are discharged before they have fully recovered their strength and mobility, and the transition from hospital to home can feel daunting.
Our post-hospital rehabilitation service bridges this gap. We begin treatment promptly after your discharge, ensuring continuity of care and preventing the loss of progress made during your inpatient stay.
Addressing Hospital Deconditioning
Even a short hospital stay can lead to significant loss of muscle strength and fitness — a phenomenon known as deconditioning. Studies show that older adults can lose up to 5 per cent of their muscle strength for each day of bed rest.
Our physiotherapists work to reverse this deconditioning through a structured, progressive exercise programme that safely rebuilds your strength, endurance, and mobility.
Reducing Readmission Risk
Hospital readmission within 30 days of discharge is a significant concern, particularly for elderly patients. Our home rehabilitation programme is designed to reduce this risk by ensuring patients are mobilising safely, managing their condition effectively, and progressing towards independence.
We coordinate with your hospital team, GP, and community services to ensure seamless, joined-up care.

"Rob is very professional and knowledgeable, really helped my husband (84) who was hospitalised for over 4 weeks and didn't get out of bed. He's got him up and running (not quite!). Hoping to move from the zimmer frame to the walking stick very soon. Would highly recommend, thank you Rob."
Dot Watson
"Rob was absolutely brilliant right from the first session. I had a very complex injury which he was able to advise on in a non-judgmental way. He is extremely knowledgeable, helpful and was able to make so many individualised recommendations in terms of exercises and equipment. The support and advice he provided has made a HUGE difference and I really can't recommend enough!"
Lauren Lovett
"Rob is a brilliant physiotherapist. He really helped me after a broken hip. Good advice and a really helpful app with all my exercises on it. I would definitely recommend him. Excellent treatment, thanks Rob."

"Rob assessed the problem with my knee which was weak and stiff following an injury. As a result of Rob's treatment and exercise programme my knee is considerably improved and I will continue with his exercises until I achieve a complete recovery.
