Deconditioning in Elderly Adults: Causes and Recovery

Deconditioning — the loss of physical function that occurs during periods of inactivity — is one of the most significant threats to older adults' health. It can occur during hospital admission, prolonged illness, or even a period of reduced activity at home. The effects are rapid and severe: older adults can lose up to five per cent of muscle strength per day of bed rest.

The causes extend beyond simple inactivity. During illness, the body's stress response increases muscle breakdown. Reduced appetite and altered nutrition during illness compound the problem. Disrupted sleep patterns, medications, and the psychological impact of illness all contribute to accelerated physical decline.

The consequences of deconditioning include weakness, impaired balance, increased fall risk, and reduced ability to perform daily activities. This creates a dangerous cycle: weakness leads to reduced activity, which causes further deconditioning, which leads to more weakness.

Reversing deconditioning requires structured, progressive rehabilitation. The process begins with gentle mobilisation and gradually increases in intensity as strength returns. Resistance exercises rebuild lost muscle tissue, while balance training addresses the fall risk that accompanies weakness.
